Gum Diseases

Gum disease is an infection of the tissues that surround and support your teeth. It is a major cause of tooth loss in adults. Because gum disease is usually painless, one may not know that they have it. Also referred to as periodontal disease, gum disease is caused by plaque, the sticky film of bacteria that is constantly forming on our teeth. Plaque that is not removed can harden and form "tartar" that brushing doesn't clean. Only a professional cleaning by a dentist or dental hygienist can remove tartar which should be regularly done as a schedule every 6 months.

Gingivitis

Gingivitis is a mild form of gum disease that can usually be reversed with daily brushing and flossing, and regular cleaning by a dentist or dental hygienist. This form of gum disease does not include any loss of bone and tissue that hold teeth in place.

Periodontitis

When gingivitis is not treated, it can advance to "periodontitis" (which means "inflammation around the tooth.") If not treated, the bones, gums, and tissue that support the teeth are destroyed. The teeth may eventually become loose and have to be removed.

Symptoms of gum disease are:

1. Bad breath that won't go away
2. Red or swollen gums
3. Tender or bleeding gums
4. Painful chewing
5. Loose teeth
6. Sensitive teeth
7. Receding gums or longer appearing teeth
8. Change in the fitting of your existing denture

How is gum disease treated?

Non-Surgical Treatments (Scaling)

Scaling means scraping off the tartar from above and below the gum line. It is generally a painless procedure. Root planing is similar to scaling but it involves cleaning of deposits and rough spots on the tooth root where the germs gather, and helps remove bacteria that contribute to the disease. These procedures are performed by periodontist (Gum specialist or hygienist) by ultrasonic scalers ideally speaking once in 6 months. Following this polishing of your teeth is done which will give a fine luster to your teeth. Occasionally medications may be prescribed depending upon the inflammatory status of the conditions.

Surgical Treatments (Flap Surgery)

If the inflammation persists even after the scaling and root planning procedures, then surgical approach might be necessary. These are minor surgical procedures performed under local anesthesia where the gums will be lifted back and cleaning of the underlying structures is done. The gums are then sutured back in place so that the tissue fits snugly around the tooth again. After surgery the gums will heal and fit more tightly around the tooth. Sometimes when the disease has caused loss of bone (holes in the supporting bone) then the periodontist might add regenerative materials in those defects, which are called bone grafts and membranes. If the disease has caused receding gums then soft tissue grafting can also be considered. Success rate of these procedures vary among individuals.

Relationship between gum disease and other systemic diseases

Now research studies have shown that there exists a strong correlation between various systemic diseases such as cardiovascular, diabetes mellitus and even premature birth with low weight babies in pregnant women who have gum disease.

Some factors increase the risk of developing gum disease are

1) poor oral hygiene

2) smoking or chewing tobacco

3) genetics

4) crooked teeth that are hard to keep clean

5) pregnancy

6) diabetes

7) medications including steroids, certain types of anti-epilepsy drugs, cancer therapy drugs, some calcium channel blockers and oral contraceptives

Can gum disease be prevented?

Gingivitis can be controlled and treated with good oral hygiene and regular professional cleaning. Tips for healthy mouth are

1. Brush and floss every day to remove the bacteria that cause gum disease.
2. See a dentist at Delhi Dental Hub at least once a year for checkups, or more frequently if you have any of the warning signs or risk factors mentioned above.
